Below is the Handbook for the 2020/2021 season outlining our expectations of the Varsity, Junior High, Youth & Youth Prep dancers in our program. All prospective dancers, and their parents, must sign this document indicating an understanding of our expectations and agreeing to their terms for the new season before they are permitted to participate in the final team selection/placement process. Please view the mandatory Lindsay’s Law video prior to signing the form below for tryouts/sign-ups. This is a State of Ohio requirement for all minors participating in organized sports/activities.

Seton Students

If you are going to be a potential new member of Varsity or Junior High (freshmen) for the 2020/2021 season we need the ImPact testing form below turned in at or prior to tryouts. ONLY the girls chosen for our teams will actually have the 1 time testing done by our Seton Trainer/TriHealth.

NOTE: If you were on the Varsity team in the 2019/2020 season your 1 time testing has been completed already.

TriHealth/Seton ImPACT Testing Consent Form

All Seton student athletes MUST have a current (within the past year) OHSAA Physical Form on file at Seton at all times. Freshmen/New Dancers for the 2020/2021 season must have their form turned in BEFORE tryouts start on May 11, 2020.

2020/2021 OHSAA Pre-Participation Physical Form

NOTE: Seton will offer pre-participation physicals for students on TBD. We will post the re-scheduled date ASAP due to COVID-19. There is a $15 charge for this service. They will accept cash or check payable to Seton High School. This is a great option for those needing to renew their pre-participation physical from the 19/20 season. All new/transfer students for the 2020/2021 school year  will need to have their pre-physical done before tryouts begin on May 11th.

All new students must also have an account on Final Form for the 2020/2021 school year but that will not be activated for the new year until after 6/1/20. More details to follow.
